Eligibility Guidelines:

1. Candidate must be between the ages of 14-22 years of age

2. Must complete the life skills course and exam online

3. Must have a parent or guardian's permission to submit application (under 18 yrs)

4. Complete the life plan questionnaire and submit the request form
